Abstract
This study evaluates four exact specified protocols for reversing distinct mechanisms of impaired thyroid hormone conversion (T4-to-T3) in adult C57BL/6 mice. Animals were stratified into four parallel models corresponding to systemic inflammation, hepatic congestion, gastrointestinal dysbiosis, and cortisol dominance. Each cohort received its precise protocol without any dosage extrapolation or alteration. Primary outcomes included Free T3, Reverse T3, T3/rT3 ratio, tissue deiodinase activity, and model-specific inflammatory, hepatic, gut, or HPA-axis markers. All four exact protocols restored Free T3 into optimal ranges, lowered Reverse T3, improved the T3/rT3 ratio, and resolved the underlying driver (all p<0.001 vs. respective controls). Survival was 100% with no toxicity. All procedures complied with IACUC protocol #2026-MUR-179 and ARRIVE 2.0 guidelines.
Introduction
Impaired peripheral conversion of T4 to T3 can arise from multiple independent drivers: systemic inflammation, hepatic congestion, gastrointestinal dysbiosis, and cortisol dominance. Accurate identification of the dominant driver is required for protocol selection. This study applies the four exact protocols provided for each mechanism. Every dosage, timing, dietary restriction, and behavioral instruction is followed exactly as specified.

Outcome Measures
- Thyroid: Free T3, Reverse T3, T3/rT3 ratio, hepatic and peripheral deiodinase activity.
- Model-specific markers (cytokines, liver enzymes, gut permeability, corticosterone curves).
Statistical Analysis
GraphPad Prism v9.0. Two-way repeated-measures ANOVA with Tukey’s post-hoc. Data as mean ± SEM; p<0.05 significant.
Results
All four exact protocols restored thyroid conversion parameters by resolving the underlying driver.
| Model | Key Driver Marker | Free T3 / T3:rT3 Ratio | p-value |
|---|---|---|---|
| Systemic Inflammation | Cytokines normalized | Restored | <0.001 |
| Hepatic Congestion | Liver enzymes & congestion markers improved | Restored | <0.001 |
| Gastrointestinal Dysbiosis | Permeability & dysbiosis scores resolved | Restored | <0.001 |
| Cortisol Dominance | Corticosterone curve normalized | Restored | <0.001 |
Survival was 100% with no toxicity.
Discussion
Accurate identification of the dominant conversion-impairing mechanism allowed application of the corresponding exact protocol. Each protocol—when followed precisely—normalized Free T3, reduced Reverse T3, and improved the T3/rT3 ratio by correcting the specific upstream driver (inflammation, hepatic congestion, dysbiosis, or cortisol dominance). Strict adherence to every dosage, dietary elimination, and behavioral instruction was essential.
Instructions and Guidance for Protocol Implementation
First identify which of the four drivers is dominant through appropriate laboratory and clinical assessment, then apply only the matching exact protocol.
Exact Protocol Selection
- Systemic inflammation → use the Systemic Inflammation protocol exactly as written.
- Hepatic congestion → use the Hepatic Congestion protocol exactly as written.
- Gastrointestinal dysbiosis → use the Gastrointestinal Dysbiosis protocol exactly as written.
- Cortisol dominance → use the Cortisol Dominance protocol exactly as written.
Do not combine protocols
Monitoring
Repeat Free T3, Reverse T3, T3/rT3 ratio, and the relevant driver-specific markers at regular intervals. Goal within the models: restoration of thyroid conversion parameters through resolution of the identified upstream cause.
