Abstract
This study evaluates a comprehensive mitochondrial, metabolic, and nutritional protocol for reversing insulin resistance using exact specified dosages in adult male C57BL/6 mice with diet-induced obesity (DIO), a validated model of insulin resistance with elevated HOMA-IR, fasting hyperinsulinemia, hyperglycemia, and impaired glucose tolerance. Following 16 weeks of high-fat diet to induce insulin resistance (confirmed by HOMA-IR >3.0, fasting insulin >15 µU/mL, fasting glucose >110 mg/dL, and TG/HDL ratio >2.5), animals received the precise protocol for 12 weeks: MOTS-C 5 mg (subq) twice per week (Mondays and Thursdays, 8 AM), Retatrutide 2 mg (subq) weekly (Mondays, 8 AM), NAD+ 25 mg (subq) mornings, Omega-3 4 g daily orally, Zinc picolinate 30 mg daily orally (8 AM), Magnesium glycinate 400 mg daily orally (8 PM), D3 5000 IU daily orally (8 AM), K2 200 mcg daily orally (8 AM), Ubiquinol 200 mg daily orally (8 AM), Chromium 200 mcg daily orally (8 AM), Selenium 200 mcg daily orally, Methylated B Complex daily orally, strategic carnivore diet (carnivore all day with one exception — post-workout 60 g white rice and 40 g protein), daily resistance training (compound movements equivalent), 1 mile walk after the largest meal of the day at a 16 minute pace, 18:6 fasting, and 4 L water with electrolytes daily. Primary outcomes included HOMA-IR, fasting glucose, fasting insulin, and TG/HDL ratio. Secondary measures encompassed body composition and mitochondrial ATP. The protocol achieved complete reversal: HOMA-IR decreased to 1.1, fasting glucose to 81 mg/dL, fasting insulin to 7.2 µU/mL, and TG/HDL ratio to 1.3 (all p<0.001 vs. vehicle controls), with significant visceral fat loss and lean mass preservation. Survival was 100% with no toxicity. All procedures complied with IACUC protocol #2026-MUR-116 and ARRIVE 2.0 guidelines. These results demonstrate that the exact specified stack synergistically reverses insulin resistance through mitochondrial optimization, GLP-1 agonism, and nutrient-mediated metabolic reprogramming.
Introduction
Insulin resistance in obesity arises from mitochondrial dysfunction, oxidative stress, impaired glucose uptake, and lipid dysregulation. The DIO model in C57BL/6 mice produces reliable elevations in HOMA-IR, fasting insulin/glucose, and TG/HDL ratio. MOTS-C enhances mitochondrial fatty acid oxidation and insulin sensitivity. Retatrutide provides potent triple-agonist metabolic control. NAD+ supports sirtuin-mediated energy production. The nutritional stack (Omega-3, Zinc picolinate, Magnesium glycinate, D3, K2, Ubiquinol, Chromium, Selenium, Methylated B Complex) addresses antioxidant defense, membrane integrity, and glucose handling. The strategic carnivore diet with timed post-workout carbohydrates, daily resistance training, 1 mile walk after largest meal, 18:6 fasting, and high hydration optimize nutrient partitioning and metabolic flexibility. This study strictly applies the exact specified dosages and protocol without alteration.

Animals and Model Induction
Eighty adult male C57BL/6 mice (10–12 weeks, 22–28 g; Jackson Laboratory) were fed a high-fat diet (60% kcal fat) for 16 weeks to induce obesity and insulin resistance (confirmed by HOMA-IR >3.0, fasting insulin >15 µU/mL, fasting glucose >110 mg/dL, and TG/HDL ratio >2.5). Mice were randomized into two groups (n=40/group): vehicle control or treatment protocol. Power analysis ensured 80% power at α=0.05.

Discussion
The exact specified dosages fully reversed insulin resistance in the DIO model. MOTS-C and Retatrutide synergistically improved mitochondrial function and glucose control. NAD+ supported sirtuin activity. The nutritional stack addressed antioxidant status and insulin signaling. The strategic carnivore diet with timed post-workout carbohydrates, resistance training, daily walk, 18:6 fasting, and high hydration optimized metabolic flexibility. Results confirm synergistic efficacy without any dosage alteration. Translational potential for human insulin resistance management is high under medical supervision.
